We are looking for someone to join the Bristol Rough Sleeping Service in the role of Hospital Discharge Coordinator where you will work across two main Bristol hospitals to provide advocacy, support and quality discharge interventions for people experiencing homelessness while attending hospital.
You will work with patients experiencing homelessness, hospital staff, local authorities, housing providers and other organisations to improve communication, understanding, discharge practices and access to services for this often excluded group.
The ability to deal with complex behaviours in relation to people and the empathy to provide person centred support to people who may be in a difficult personal situation.
