The Sirona TB Nurse Team work collaboratively with patients and a wide range of colleagues to provide high quality care for people affected by TB across the Bristol, North Somerset and South Gloucestershire localities. We are proud of our high treatment completion rates and the feedback we receive from patients.
The role is primarily based in the community and includes patient centred case management for those on TB treatment, supporting diagnosis of TB, LTBI screening programmes, running clinics, professional education, and contact tracing.We have strong links with public health and often work with UKHSA.
Main duties of the job
- Takes lead responsibility for the case-management of TB cases, assessing need and implementing and evaluating individual programmes of care and support for people being treated for TB.
- Work in community clinics, patients' own homes.
- Maintaining close liaison with GPs and hospital services.
- Responsible for collating accurate data and meeting performance targets, contributing to research and audit.
- Recognise patient perspectives, seek their views and feed those into service development
- Provide education programmes and presentations on TB and related issues within Sirona and the wider health/social community as required.
- Attend TB clinics in the acute trusts alongside own patient list.
- Communicate effectively with GPs, patients and use IT systems effectively to enable care.

Qualifications
Essential
- Registered with the NMC (Level 1)
- Evidence of relevant continuing professional development, evidence of post registration qualification in community health or tuberculosis or willingness to undertake.
Skills and experience
Essential
- Expert assessment and case management skills to support patients adhere to lengthy and difficult treatment regimes. Providing individualised patient care based on the principles of evidence based practice. Acting as patient's advocate as required.
- Expert skills to communicate effectively with the team, client groups and patients from a wide range of social and cultural backgrounds, including those who are socially excluded.
- Ability to manage a complex caseload.
- Experience or interest in working with people being treated for Tuberculosis (TB).
- Knowledge of commissioner provider issues and how these impact on service provision and an understanding of key national and local priorities for community nursing.
- oExperience of working across organisational boundaries within health or social care.
