Job Requirements/Description

Come and join a supportive team providing high quality rehabilitation for people after stroke in the community. This is a fixed term contract for a year to cover a secondment and will allow you to develop your skills and knowledge with support from stroke specialists in the multidisciplinary team. You will be providing holistic assessment and rehabilitation of stroke related issues such as cognition, vision, return to work, fatigue, upper limb and working towards people's functional goals around activities of daily living including personal care, meal preparation, work and leisure activities and accessing their local community. Formal and informal supervision is provided and opportunities for CPD through in service training and external courses where appropriate.

Main duties of the job

  • With support be responsible for holistic assessment, implementation and evaluation of care for patients following a stroke
  • Use clinical reasoning skills to assess, plan, implement and evaluate, patient- centred intervention programmes using evidence based practice.
  • Collaborate and negotiate with service users and carers in identifying appropriate and achievable goals which will maximise quality of life and independence.
  • Facilitate appropriate referrals to other services based on your assessment to ensure holistic patient care
  • Work within a multi-disciplinary team, to provide integrated skills and sharing knowledge providing holistic and specialist skills supported by a robust competency framework
  • To apply theoretical and working knowledge of medical, neurological, surgical, psychological, trauma and terminal conditions, then work with the implications of these on the quality of life of patients and carers.
  • To use occupational; therapy skills, techniques in functional and standardised assessment & interventions as an autonomous practitioner
  • Create person centred, goal based treatment plans using outcome based approach which can be delegated and supported by Stroke colleagues.
  • To assess and arrange the provision of equipment and/or adaptations from appropriate agencies ensuring this is provided in good working order and fitted correctly; reporting any issues appropriately
  • Supervise members of the team including peers, students and unregistered workforce

Qualifications

Essential

  • Recognised Occupational Therapy qualification
  • HCPC registration

Knowledge

Essential

  • Awareness of the role of OT in stroke rehabilitation
  • Able to work autonomously with distant support
  • Awareness of guidelines and policies around stroke rehab and wider patient care

Experience

Essential

  • Experience of working as an OT or OT student in a relevant clinical area

Desirable

  • Experience of community working

Skills

Essential

  • Able to provide assessment and rehabiliation as an autonomous clinician

Desirable

  • Evidence of CPD in stroke rehabilitation
